Open my.chirocongress.org in Safari on your iPhone
Tap the Action icon (box with up arrow icon - pictured above)
Scroll to the right thru the mobile device icons at the bottom of your screen and select the 'Add to Home Screen' (plus sign icon - pictured above)
When given the option, change the name to 'ChiroConnect' (backspace thru the text that is listed there already)
Click on 'Add' in upper right hand corner
You should now have an icon that displays on your phone that looks like below.
